(ON Only) The Summary and Goals Tab

The Summary/Goals Tab allows you to document the outcome of the MedsCheck. In this tab you can add multiple topics in regards to any summary or goals the patient may have.

Upon completion of the medication review, the fields in this tab automatically populate the Summary and Goals section of the Pharmacist Worksheet form and the Patient Take-Home Summary form. Pharmacists may provide patients with the Patient Take-Home Summary that is intended to further engage patients in identifying ways to build added awareness in their drug therapy and help to identify therapy-related goals.

Note: If you are completing a diabetes medication review, the Patient Take-Home Summary form will be titled Diabetes Education Take-Home Summary form.

To complete this tab, do the following:

1. In the Summary and Goals section, enter the patient's goals, as discussed during the MedsCheck.
2. In the Notes section, enter any thoughts or suggestions you wish to include. Select the Add Note button once you have completed your note.
3. If you wish to add further information or additional goals, select the button. A new Topic tab will appear. Proceed to complete the fields on this tab as needed.
